Hey Nightshade!

When a gem replaces an organ, it replicates the function but would not replicate a disease (or other type of 'malfunction'); its primary role is just to maintain the life of the organism in such a case. I didn't read much on it because I'm short on time right now but Hepatogenous seemed related to the liver; was Type IV also specifically related to the liver, or just caused by an unknown agent? If it didn't need to be related to the liver, you could still go with Type IV (I was reading here: https://www.merckvetmanual.com/integumentary-system/photosensitization/overview-of-photosensitization#v3281783)

Oh, yeah, I was reading there, too! Type IV is caused by an unknown agent, yes. Hikari's character page has been edited to reflect this change.
